We'll ignore that Lee's leg muscles should have been torn to shreds carrying those things a long time ago.

I remember watching this scene for the first time. Everything about the build up made it obvious that those weights were going to be unreasonably heavy, and I was STILL shocked at how hard those things landed.

He obviously didn't start with that much weight. He worked his way up to it.

Naruto (and its sequels) are set in an extremely heightened reality, so this works. Anything remotely realistic though, and it falls apart fast because

1) You need a certain level of physical maturity for such training to even be an option; young Rock Lee can't have started this too long ago.

2) Based on the effects of the weights dropping on the scenery, they weigh something like 5 tons. XP No matter how low you start out, remotely realistic humans cannot do that.

3) Couldn't find a canon number for the weights, so someone feel free to let us know. :D

Holy Boom is right.

Even assuming his muscles could safely lift and carry those weights, what kind of fabric must that be to hold the weights onto the belts without being torn apart? How tightly must the belts be bound to his legs to avoid them sliding off with that weight? Wouldn't that cut off his blood flow? Why didn't his every step make the same boom they made when they landed there? What must they be made of to have such weight while still being so small? And, if these weight offer such a massive benefit, why didn't he have some on his wrists, too?

So many questions...the only answer is, "Shut up; Rock Lee is awesome; don't question him."

To release that much dust there are three possibilities...

1) the weights are so ridiculously heavy that they impact with enough force to crack the ground, pounding the pavement to dust. This has been discussed as unlikely.

2) The floor is ridiculously dusty, which might be possible given how much sand (similar to dust) has been flung around the arena in the past few minutes.

3) the "weights" contain impact-activated smoke-bombs.

The show treats (1) as the most likely... but I can't help but wonder... what if (3) is the truth?

The real question is: How did everyone up until this point survive getting kicked by him?

Clearly, he was holding back to avoid turning them into a fine, not-youthful paste.
