I remember way back in that first one-shot I was in, the DM briefly forgot Duke Harris' name, and the group tried to call him "Baron Billiam" instead.

Story time! Tell a story about forgetting an important NPC's name.

Every time.

Okay, okay, it wasn't quite that bad but everyone in my group was either too lazy or arrogant confident in their memory to take notes, and yes it did bite us in our collective butt frequently. >.>

"importatnt NPC"
^suggesting that players recognize NPCs as anything more than "The guy who gives us money," "The guy who gives us booze," or "The guy we want to kill because of... reasons. probably"

In seriousness, it happens to everyone to the point where one of our DMs doesn't even bother naming his NPCs unless we specifically ask for the name, and half the time, it ends up being the first thing someone shouts out. Thus was introduced, our good friend, never to be seen again after that session, "Molotov Kokktail" (there had been jokes earlier about specifying that someone's name was spelled with a K)

Now, the butcherings that occur of PCs' names are great, because you can't get rid of them once they stick
-Character with a thick German accent: "My name is Prince Vincent von Schnitz"
-Other player (me) at the next session: "What's your guy's name again? Shin Splints?"

-Mute character (speaks by writing notes on paper): "I am Myrrhen"
-Me IRL leaning across the table to try to read the handwriting: "I am... Moron?"
